1. A. The Secretary or the Commission, as appropriate, shall prepare an official record of each hearing.

2. B. The record shall consist of:

   1. (1) Any document giving rise to the proceeding;
   2. (2) Any request for the hearing;
   3. (3) Notices of all proceedings;
   4. (4) Any prehearing order;
   5. (5) Motions, pleadings, briefs, petitions, requests, and intermediate rulings;
   6. (6) Evidence received or considered;
   7. (7) A statement of each fact officially noticed;
   8. (8) Proffers of proof, objections, and rulings on them;
   9. (9) Proposed and final findings of fact, conclusions of law, and requested orders;
   10. (10) Matters placed on the record after an ex parte communication;
   11. (11) A recording of the hearing, any transcript of the recording, or any transcript prepared by a court reporting service; and
   12. (12) Any order.
