1. A. A provider may voluntarily surrender a child care home registration at any time by notifying the office in writing.

2. B. If a registration is surrendered pursuant to §A of this regulation:

   1. (1) The office shall note in the provider's file that the registration has been voluntarily surrendered; and
   2. (2) The provider shall return the surrendered certificate of registration to the office.
