1. A. IFSP Team Members.

   1. (1) A local lead agency shall ensure the initial IFSP team meeting and the annual IFSP team meeting to evaluate the IFSP of an infant or toddler with a disability shall be multidisciplinary and include the following participants:

      1. (a) The parents of the child;

      2. (b) The service coordinator who:

         1. (i) Has worked with the family since the initial referral of the child; or
         2. (ii) Was designated by the public agency to be responsible for implementation of the IFSP;
      3. (c) Individuals directly involved in conducting the evaluations and assessments in accordance with Regulation .05 of this chapter;
      4. (d) Individuals who will provide services to the child or the family, as determined appropriate;
      5. (e) Other family members, as requested by the parent, if feasible to do so; and
      6. (f) An advocate or individual outside the family, if the parent requests that the individual participate.

   2. (2) At a minimum, the IFSP team shall include:

      1. (a) The parent; and
      2. (b) Two or more individuals from separate professions.
   3. (3) One of the individuals listed in §A(2)(b) of this regulation shall be the child’s service coordinator.

   4. (4) If an individual listed in §A(1) of this regulation is unable to attend a meeting, arrangements shall be made for the individual's involvement through other means, including:

      1. (a) Participating in a telephone conference call;
      2. (b) Having a knowledgeable authorized representative attend the meeting; or
      3. (c) Making pertinent records available at the meeting.

2. B. IFSP Team Meetings.

   1. (1) The IFSP team meeting shall be conducted:

      1. (a) In a setting and at a time that is convenient to the family; and
      2. (b) In the native language of the family or other mode of communication used by the family, unless it is clearly not feasible to do so.
   2. (2) Written notification of the IFSP team meeting shall be provided to the family and other participants early enough before the meeting date to ensure invited individuals will be able to attend.
