1. A. Unless the Department suspends or revokes a CHW certification training program accreditation, the accreditation is effective for 3 years from the date issued.

2. B. At least 90 calendar days before the accreditation expires, the Department shall send to the accredited CHW certification training program by electronic or first-class mail to the last known electronic or physical address a renewal notice that states:

   1. (1) The date on which the current accreditation expires;
   2. (2) Any required renewal submission requirements;
   3. (3) The date by which the renewal application shall be received by the Department for the renewal to be issued; and
   4. (4) The amount of a renewal fee.
3. C. The accredited CHW certification training program shall notify the Department if a renewal notice is not received within 60 calendar days of the renewal date.

4. D. The Department shall renew the accredited CHW certification training program that:

   1. (1) Is eligible to obtain accreditation;
   2. (2) Pays to the Department a renewal fee set by the Department in regulation; and
   3. (3) Submits to the Department a renewal application on a form that the Department requires.
5. E. Renewal applications received less than 30 calendar days before the expiration date may not be processed before the expiration date.
