- J. Reactivation fee (inclusive of a reactivation fee and renewal fee) . . . $350;
- K. Duplicate license or registration fee . . . $40;
- L. Duplicate license or registration ordered at renewal . . . $20;
- M. Verification fee . . . $50;
- N. Penalty for returned checks . . . $50;
- O. Mailing labels or rosters . . . $200;
- P. Penalty for failure to maintain correct name or address with the Board . . . $100;
- Q. Continuing Education Course processing fee per submission . . . $40;
