3. C. Preapplication credentials evaluation fee ... $50;
4. D. Certification fee ... $100;
5. E. Licensure fee ... $200;
6. F. Certification and trainee renewal fee ... $200;
7. G. Licensure renewal fee ... $275;
8. H. Late renewal fee (in addition to §F or G of this regulation) ... $50;
9. I. Certification and licensure verification ... $20;
10. J. Duplicate or replacement certificate or license ... $15;
11. K. Endorsement to another jurisdiction ... $10;
12. L. Name or address change ... No fee;
13. M. Inactive status (annual) ... $50;

14. N. Reinstatement fee:

    1. (1) For certificate ... $150;
    2. (2) For license ... $200;

15. O. 2-year sponsorship of continuing education programs:

    1. (1) For organization ... $750;
    2. (2) For individual ... $300; or
    3. (3) For an individual workshop ... $150;
16. P. Review for non-preapproved CEUs for individuals (per occurrence) ... $15;
17. Q. Drug and alcohol certification upgrade fee ... $25;
18. R. Returned check fee ... $25;
19. S. Failure to notify of change of address ... $25;
20. T. Copy and scanning fee ... $2 per page; and
21. U. Out-of-State application processing fee ... $250.
