1. A. Upon receipt of notification of a major adverse reaction to a pertussis vaccine, the local health officer shall:

   1. (1) Notify and forward the morbidity report card to the Department within 24 hours; and
   2. (2) Assure that the necessary information to fill out a “Report of Adverse Event Following Immunization” is obtained.
2. B. At the time of completion, the report shall be submitted to the Department.
