1. A. The applicant shall complete an application on a form approved by the Administration and submit the application and documentation to indicate the applicant is in compliance with the requirements of this chapter.

2. B. After the Administration receives the application the Administration shall:

   1. (1) Return the application if it is incomplete; or
   2. (2) Arrange a site visit.

3. C. After the site visit, the Administration shall

   1. (1) Grant approval;
   2. (2) Grant approval with conditions; or
   3. (3) Deny approval.
