1. A. The purpose of the Commission is, through community health resources, to:

   1. (1) Increase access to health care services;
   2. (2) Redirect nonemergency emergency room use to community health resources;
   3. (3) Develop coordinated, integrated systems of community-based care; and
   4. (4) Assist patients in establishing a medical home with a community health resource.
2. B. A key objective of the Commission is to reduce health disparities in the State.
