1. A. A dentist or a dental hygienist who has been granted a site-specific retired volunteer dentist's or dental hygienist's license shall immediately surrender the license to the Board when the dentist or dental hygienist ceases to:

   1. (1) Provide volunteer dental or dental hygiene services at the institution named on the license; or
   2. (2) Be covered by malpractice insurance from or through the institution named on the license.
2. B. A dentist or a dental hygienist who has been granted a retired volunteer dentist's or dental hygienist's license that is not site-specific shall immediately surrender the license to the Board when the dentist or dental hygienist ceases to be covered by malpractice insurance.
