1. A. A candidate for licensure shall:

   1. (1) Fully complete the appropriate Board application; and
   2. (2) Pay the required fee in accordance with COMAR 10.44.20.

2. B. An application shall be returned to the applicant and the applicant may not be licensed if the application:

   1. (1) Is incomplete;
   2. (2) Does not contain all of the requested information or documentation; or
   3. (3) Is not accompanied by the required fee.
3. C. The Board shall refund the application fee, less a $50 processing fee, to a dentist or a dental hygienist who does not qualify for licensure.
