A. An individual holding an expired general license to practice dentistry may apply for reinstatement if the individual:
- (1) Submits to the Board notarized affidavits, or other evidence satisfactory to the Board, that the applicant has actively practiced dentistry within the 3 years immediately preceding the date of application for reinstatement;
- (2) Submits to the Board satisfactory proof of licensure and good standing from all states in which the applicant is currently licensed;
- (3) Submits to the Board an application for reinstatement on a form provided by the Board;
- (4) Is otherwise entitled to be licensed;
- (5) Pays the reinstatement fee specified in COMAR 10.44.20;
- (6) Has fulfilled the continuing education requirements of the Board; and
- (7) Passes a written Maryland Law Examination given by the Board with at least a score of 75.
B. An individual holding an expired dental or dental hygiene teacher's license may apply for reinstatement if the individual:
- (1) Submits to the Board satisfactory proof of licensure and good standing from all states in which the applicant is currently licensed;
- (2) Submits to the Board an application for reinstatement on a form provided by the Board;
- (3) Is otherwise entitled to be licensed;
- (4) Pays the reinstatement fee specified in COMAR 10.44.20;
- (5) Has fulfilled the continuing education requirements of the Board; and
- (6) Passes a written Maryland Law Examination given by the Board with at least a score of 75.
C. An individual holding an expired general license to practice dental hygiene may apply for reinstatement if the individual:
- (1) Submits to the Board notarized affidavits, or other evidence satisfactory to the Board, that the applicant has actively practiced dental hygiene within the 3 years immediately preceding the date of application for reinstatement;
- (2) Submits to the Board satisfactory proof of licensure and good standing from all states in which the applicant is currently licensed;
- (3) Submits to the Board an application for reinstatement on a form provided by the Board;
- (4) Is otherwise entitled to be licensed;
- (5) Pays the reinstatement fee specified in COMAR 10.44.20;
- (6) Has fulfilled the continuing education requirements of the Board; and
- (7) Passes a written Maryland Law Examination given by the Board with at least a score of 75.
- D. An individual holding a license to practice dentistry or dental hygiene which has been expired for more than 3 years may not apply for reinstatement under this regulation.
