A pharmacist from a primary pharmacy may permanently transfer a prescription order to a secondary pharmacy to be dispensed to a specific patient if the pharmacist transferring the prescription from the primary pharmacy indicates on the prescription, within the prescription computer database and within any appropriate other records used for dispensing:

1. A. That the prescription has been permanently transferred;
2. B. The name of the secondary pharmacy;
3. C. The name of the pharmacist who transferred the prescription to the secondary pharmacy;
4. D. The name of the pharmacist at the secondary pharmacy to whom the prescription was transferred if the transfer occurred in an oral manner; and
5. E. The date on which the prescription was transferred to the secondary pharmacy.
