1. A. A licensed assisted living manager shall give the Board written or electronic notice within 30 days of a change of:

   1. (1) Mailing address;
   2. (2) Legal name; or
   3. (3) Email address.
2. B. Failure to notify the Board of any change of mailing address, legal name, or email address within 30 days of the change may result in a fine as set forth under COMAR 10.33.05.02D.
