1. A. The notice of failure to file a report and any final disposition under this chapter are public documents.

2. B. The notice of failure to file a report and any final disposition shall redact:

   1. (1) The name of a patient and any other patient-identifying information; and
   2. (2) Records of a health care provider’s or a naturopathic doctor’s physical, mental, or emotional condition, except to the extent necessary to identify the deficient practice or condition that caused an entity to make a change as defined in Regulation .02 of this chapter.
