To qualify for registration, a physician shall:

1. A. Complete an application on a form supplied by the Board, enclosing:

   1. (1) Required supplementary materials, which are in English, and
   2. (2) English translations of certified, required documentation with costs borne by the applicant;
2. B. Pay a registration fee of $150;
3. C. Submit a current, signed passport-sized photograph;
4. D. Be a licensed physician; and
5. E. Submit evidence of having successfully completed 200 hours of training in a medical acupuncture program or course of study, including any required examinations.
