The Secretary's designee shall:

1. A. Maintain the official record of the hearing;

2. B. Include in the official record of the hearing:

   1. (1) Official correspondence regarding the hearing, including the request for the hearing and the Secretary's response,
   2. (2) Written material submitted by the parties,
   3. (3) Evidence admitted by the Secretary's designee,
   4. (4) A tape recording of the hearing, and
   5. (5) The decision of the Secretary's designee.
