An operator shall ensure that:

1. A. Any reportable condition described in Regulation .06 of this chapter is reported to the Department on a form prescribed by the Department;

2. B. Each report form is:

   1. (1) Completed in duplicate; and
   2. (2) Signed and dated by the person completing the form;
3. C. The original report form is maintained for at least 3 years; and
4. D. A copy of the report is forwarded to the Department with personal identifiers removed to maintain confidentiality.
