Eligibility for ACIS services under this chapter is limited to individuals who meet the following health and housing criteria:

1. A. The individual is a participant that uses Maryland Medical Assistance;

2. B. The individual meets at least one of the following health criteria:

   1. (1) Repeated emergency department visits;
   2. (2) Repeated hospital admissions in the past year; or
   3. (3) Two or more chronic conditions as defined in §1945(h)(2) of the Social Security Act; and

3. C. The individual meets at least one of the following housing criteria:

   1. (1) Is homeless, chronically homeless, or at risk of homelessness as defined in 24 CFR §578.3; or
   2. (2) Is at risk of institutional placement.
