1. A. The Division may mandate the use of approved templates and forms by EPIPs, employers, and claimants including:

   1. (1) Employer notice to employee templates;

   2. (2) Claims:

      1. (a) Claim application form;
      2. (b) Certification of qualifying event forms;
      3. (c) Proof of relationship template;
      4. (d) Good cause exemption form; and
      5. (e) Intermittent leave use template; and

   3. (3) Dispute Resolution:

      1. (a) Request forms;
      2. (b) Reconsideration scheduling template;
      3. (c) Decision templates; and
      4. (d) Good cause exemption form.
2. B. The Division shall make mandated templates and forms available for download from its website.
3. C. Any changes to the fields on the templates or forms mandated by the Division shall be provided to the Division no less than 30 days before the changed template or form would go into effect.
