1. A. If a registrant fails to renew a license for any reason, no household goods mover services may be offered or provided until the registration is reinstated.

2. B. A request for reinstatement shall:

   1. (1) Be in writing;
   2. (2) Be on the form required by the Department; and
   3. (3) Include payment of the reinstatement fee.
