A computerized monitoring system shall be equipped with an alarm that will activate when:

1. A. The stack flue gas reaches a high temperature;
2. B. The burner status indicator indicates the burner is “off”;
3. C. The flame safety control status indicates a “flame failure”; and

4. D. One of the following occurs, as applicable:

   1. (1) For steam boilers, the:

      1. (a) Steam reaches low or high pressure; or
      2. (b) Water reaches low or high level;
   2. (2) For a hot water boiler operating at 30 to 299 horsepower, the water reaches low or high temperature; or

   3. (3) For a hot water boiler operating at 300 or more horsepower the water reaches low or high:

      1. (a) Pressure;
      2. (b) Temperature; or
      3. (c) Level.
