1. A. The Inflatable Amusement Attraction Daily Inspection and Daily Pre-Opening Checklist shall be:

   1. (1) Signed by the person performing the inspection immediately upon completion of the inspection;
   2. (2) Kept with the inflatable amusement attraction or at an on-site office; and
   3. (3) Maintained for at least 30 days.

2. B. Identification Records. An owner shall maintain a record of the name and identification number of each operator and attendant with the following information:

   1. (1) Inflatable amusement attraction that the operator and attendant operated; and
   2. (2) The date and location of operation.

3. C. Record of Training.

   1. (1) Each owner shall:

      1. (a) Maintain a record of training provided to each operator and attendant; and
      2. (b) Provide the Commissioner with a copy of the record of training on request.

   2. (2) The training record shall indicate the following for each operator and attendant:

      1. (a) The name or identification number;
      2. (b) A detailed description of the training provided; and
      3. (c) A list of all the inflatable amusement attractions for which the operator and attendant received training.

4. D. Written Logs.

   1. (1) An owner shall ensure that a written log is maintained and:

      1. (a) Written log requirements:

         1. (i) Contain any reports from riders complaining of adverse physical effects or operating inconsistencies occurring during the operation of an amusement attraction; and
         2. (ii) Are submitted to the Commissioner when multiple reports of a particular nature may indicate a pattern are shown in the log; and
      2. (b) Contains weather reports that reflect review of the weather forecast prior to each set up of the inflatable amusement attraction including the date, time, location of operation, and description of weather conditions.
   2. (2) A copy of the written logs required by this section shall be made available to the Commissioner upon request.
