1. A. To be considered for participation in the Program, the noncustodial parent shall:

   1. (1) Submit to the Administration a completed application on a form provided by the Administration;
   2. (2) Provide all information specified in the application form; and
   3. (3) Include documentation as required by the Administration.
2. B. The Administration shall provide a written decision to the noncustodial parent not later than 60 days after the receipt of the application.
