1. A. Forms.

   1. (1) The Department shall provide to the administering agencies the forms necessary to carry out the Program.
   2. (2) The administering agencies shall provide Program service forms to the emergency food providers.

2. B. Reports.

   1. (1) An emergency food provider shall submit a report annually, and other reports as required, to the Department and the administering agency detailing:

      1. (a) How the grant was used; and
      2. (b) The units of service provided.

   2. (2) An administering agency shall submit a report annually, and other reports as required, to the Department detailing for each jurisdiction for which the administering agency is responsible:

      1. (a) How the grant was used in each jurisdiction; and
      2. (b) The units of service provided in each jurisdiction.

   3. (3) The Department shall submit annually to the State Advisory Council on Hunger a report detailing:

      1. (a) The total funds appropriated to the Program for the current and prior fiscal years, including the amount allocated for:

         1. (i) Administrative costs;
         2. (ii) Each administering agency; and
         3. (iii) Emergency food providers in each jurisdiction; and
      2. (b) The units of service provided in each jurisdiction.
