1. A. An application for a food desert designation shall be submitted by a local government or local governments.
2. B. An application for a food desert designation shall be submitted to the Department on forms prescribed by the Department and in accordance with the schedule that the Department establishes.

3. C. An application for a designation shall provide:

   1. (1) A detailed map and description of the proposed food desert demonstrating the proposed areas within a priority funding area;

   2. (2) Information and data on the:

      1. (a) Availability of fresh fruit, vegetables, and other healthy foods in the area;
      2. (b) Income levels of local residents; and
      3. (c) Transportation needs of local residents and the availability of public transportation;
   3. (3) Any comments the applicant has received from the local governments or community; and
   4. (4) Any other information that the Department considers relevant.
