1. (a) If, during any license year, there is any change in the information that a licensee gave the Administration in obtaining a license under this title, the licensee shall report the change to the Administration within 30 days after the change occurs.
2. (b) If, during any license year, there is any change in the information that a licensee gave the Administration in retaining a license under this title, the licensee shall report the change to the Administration within 30 days after the change occurs.
3. (c) The report under this section shall be made on the form that the Administration requires.
4. (d) The licensee shall sign the form and certify that the information given in it is true.
