Md. Code Ann., State Gov't § 9-2424 – Reinstatement

(1) applies to the Board for reinstatement of the license within the period of time established by the Board;
(2) meets the renewal requirements of § 9-2423 of this subtitle; and
(3) pays to the Board the renewal fee and the reinstatement fee set by the Board.

The Board shall reinstate the license of an individual who for any reason has failed to renew the license if the individual:
