1. (a) An applicant for a certificate shall:

   1. (1) submit to the State Fire Marshal an application on the form the State Fire Marshal provides;
   2. (2) submit all documents that the State Fire Marshal requires; and
   3. (3) pay to the State Fire Marshal an application fee of $100.
2. (b) The application form provided by the State Fire Marshal shall contain a statement advising the applicant that willfully making a false statement on an application is a misdemeanor, subject to a fine or imprisonment or both, as provided in § 12-616 of this subtitle.
