1. (1) the policy contains the provisions and conforms to the requirements that the Commissioner sets by regulation; and
2. (2) a copy of the form for the policy has been filed under § 12-203 of this article and approved by the Commissioner.

A policy of wholesale life insurance may not be delivered or issued for delivery in the State unless:
