1. (a) An applicant for a limited lines license as a self-service storage producer shall file a written application with the Commissioner in the form the Commissioner requires.

2. (b) The application shall include:

   1. (1) the physical address of the company headquarters of the self-service storage producer; and
   2. (2) a list of all self-service storage facilities where the self-service storage producer will conduct business under the limited lines license.
