1. (a) An application for assistance under this subtitle shall be made:

   1. (1) to the local department of the county where the applicant resides; and
   2. (2) in the form and manner that the Administration requires.

2. (b) An application for assistance under this subtitle shall include a medical form that:

   1. (1) contains the name and estimated duration of the applicant's impairment; and
   2. (2) is signed by a licensed health care provider.
