1. (a) Unless the Board agrees to accept the surrender of a license, a licensed electrologist or licensed electrology instructor may not surrender the license.
2. (b) The Board may require terms and conditions on an agreement with the licensed electrologist or licensed electrology instructor to accept surrender of the license.
3. (c) An agreement to accept the surrender of a license is a final order of the Board and is a public record.
