1. (a) An applicant for a license shall submit an application to the Secretary.

2. (b) The application:

   1. (1) Shall be on the form that the Secretary requires;
   2. (2) Shall be signed and verified by the individual; and

   3. (3) Shall include:

      1. (i) The name of the applicant;
      2. (ii) A statement that the applicant meets the requirements of this subtitle;
      3. (iii) The location of the proposed hospice;
      4. (iv) The name of the individual who is to be the medical director of the hospice care program; and
      5. (v) Any other information that the Secretary requires.
3. (c) Repealed by Acts 2018, c. 661, § 1, eff. July 1, 2018.
