1. (a) There is a Community Integrated Medical Home Program.

2. (b) The mission of the Community Integrated Medical Home Program is to:

   1. (1) Keep Maryland families healthy through the use of innovative mapping tools that allow better targeting of resources to those in need;
   2. (2) Coordinate comprehensive services provided by a participating patient centered medical home with public health resources in local communities across the State; and
   3. (3) Provide complementary support for qualified individuals between office visits.
3. (c) The Community Integrated Medical Home Program shall be administered jointly by the Commission and the Department.
