1. (a) The Secretary shall issue a permit to any applicant who meets the requirements of this subtitle.

2. (b) The Secretary shall include on each permit that the Secretary issues:

   1. (1) The name of the tissue bank;
   2. (2) The name of its director;
   3. (3) The name of its owner; and
   4. (4) The classes of services that the tissue bank may offer.
