1. (a) After appointment, letters shall be issued to the personal representative by the register.

2. (b) Letters shall contain:

   1. (1) The name and location of the court or register by whom appointment was made;
   2. (2) The name of the decedent and the personal representative;
   3. (3) The date of the representative's appointment;
   4. (4) The date of probate of the will admitted to probate in the proceeding;
   5. (5) The signature of the register and the seal of the court; and
   6. (6) The date the certificate was issued.
