1. (1) Notify the Department promptly of:

   1. (i) The name of the radioactive substance involved;
   2. (ii) The quantity of the radioactive substance released;
   3. (iii) The intensity of the radiation released; and
   4. (iv) The composition of all radionuclides of the radioactive substance in terms of constituent weights; and
2. (2) Provide any additional information that the Department requests.

If any radiation escapes from a low-level nuclear facility into the environment, the low-level nuclear waste facility permit holder shall:
