1. (a) This section does not apply to any controlled hazardous substance that is:

   1. (1) Used for residential purposes; or
   2. (2) Regulated by the State Department of Agriculture.
2. (b) Any person who generates, uses, treats, stores, or disposes of a controlled hazardous substance at any time after 6 months from the date the Department issues a rule or regulation that identifies the hazardous substance to be a controlled hazardous substance shall file a report with the Department.

3. (c) The report required by this section:

   1. (1) Shall be filed within a time set by the Department;
   2. (2) Shall be on the form provided by the Department; and

   3. (3) Shall include the following information about the controlled hazardous substance, if applicable:

      1. (i) Name of the hazardous substance.
      2. (ii) Volume.
      3. (iii) Quantity.
      4. (iv) Level and kind of radioactivity.
      5. (v) Half-life.
      6. (vi) Manner of management.
      7. (vii) Manner of disposal.
