1. (a) Within 30 days after an operation is abandoned or completed, the operator shall file with the Department a completion report on a form prescribed and furnished by the Secretary.

2. (b) The operator shall attach to the completion report:

   1. (1) A map of the operation certified by a surveyor or engineer that shows the boundary lines of the tract and the access to the operation from the nearest public highway;
   2. (2) The area of land affected by the deep mining operation; and
   3. (3) Details of the completed reclamation work.
