1. (1) If known, the name of the lender or the designated agent of the lender;
2. (2) If known, the last known address of the lender or the designated agent of the lender;
3. (3) A brief description of the property on loan;
4. (4) If known, the date of the loan;
5. (5) The name of the museum; and
6. (6) The name, address, and telephone number of the person or office at the museum to contact regarding the property.

In addition to any other information required under this subtitle, any notice given by a museum under this subtitle shall contain:
