1. (1) the names and addresses of the parties;
2. (2) the services to be provided;
3. (3) the number of persons to be involved in providing the services;
4. (4) the time when the services are to be provided; and
5. (5) the method and formula for compensation.

A person may not act as a fund-raising counsel unless the person's agreement with the charitable organization states:
