1. (1) a statement whether the applicant is a natural-born or naturalized citizen; or

2. (2) if the applicant is not a citizen of the United States:

   1. (i) a statement that the applicant's immigration status is in accordance with federal law; and
   2. (ii) evidence to verify the immigration status of the applicant.
