(2) Requestors who are not firearms dealers shall provide the following information at the time of the request:

1. (a) requestor's name;
2. (b) requestor's address;
3. (c) requestor's telephone number;
4. (d) requestor's email address;
5. (e) requestor's license to carry (LTC) number or firearms identification card (FID) number;
6. (f) type of firearm;
7. (g) manufacturer's name and address;
8. (h) model of the firearm;
9. (i) caliber of the firearm;
10. (j) barrel length of the firearm;
11. (k) whether the request is for a privately made firearm; and
12. (l) if the request is for a privately made firearm the means and manner of production.

(3) A dealer requesting a serial number shall provide the following information (to the extent applicable) at the time of the request:

1. (a) dealer's name;
2. (b) dealer's address;
3. (c) location of business;
4. (d) dealer's telephone number;
5. (e) dealer’s email address;
6. (f) type of firearms to be serialized;
7. (g) manufacturer's name and address;
8. (h) model of the firearm;
9. (i) caliber of the firearm;
10. (j) barrel length of the firearm;
11. (k) whether the request is for a privately made firearm; and
12. (l) if the request is for a privately made firearm, the means and manner of production.
