Any Criminal Justice Agency that collects data on a person shall submit any data it collects for the following informational fields in the format set forth in 501 CMR 18.00 to the data warehouse:

1. (a) State Identification Number (SID);
2. (b) Probation Central File Number;
3. (c) Governing Offense;
4. (d) Offense;
5. (e) Date of Offense;
6. (f) Time of Offense;
7. (g) Location of Offense;
8. (h) Offender's Address at Time of Arrest;
9. (i) Race of Offender;
10. (j) Ethnicity of Offender;
11. (k) Gender Identity;
12. (l) Assigned Sex;
13. (m) Pronouns;
14. (n) Age of Offender;
15. (o) Status of Offender as Primary Caretaker of a Child;
16. (p) Status of Offender's Reproductive Health Needs;
17. (q) Risk and Needs Assessment Scores;
18. (r) Whether the Offender has Participated and Completed Evidence-based Programs;
19. (s) Date Entering Custody;
20. (t) Date Exiting Custody;
21. (u) Offender's Address on Date of Exit Custody;
22. (v) Release Date;
23. (w) Offender's Address on Release Date;
24. (x) Actual Release from Sentence Date;
25. (y) Offender Status;
26. (z) Offense Based Tracking Number;
27. (aa) Type of Release;
28. (bb) Type of Admission;
29. (cc) Case Disposition;
30. (dd) Jail Credit from Pretrial Incarceration;
31. (ee) Earned time; and
32. (ff) Bail Amount or Reason if no Bail Set.
