At the patient's or patient's agent's request a pharmacy may dispense medications in a reusable daily dosage planner provided the following requirements are met:

1. (1) A pharmacy may not place any medication in a reusable daily dosage planner that was previously dispensed by a different pharmacy.
2. (2) The pharmacy designates a space that allows for the orderly placement of equipment, materials, and medications, for the proper preparation of reusable daily dosage planners, and for the prevention of cross-contamination.
3. (3) The pharmacy maintains policies and procedures pertaining to reusable daily dosage planners that include cleaning, labeling, dispensing, and proper hand hygiene.
4. (4) The pharmacy cleans and stores reusable daily dosage planners in a manner that prevents contamination to the pharmacy environment or reusable daily dosage planner.
5. (5) The pharmacy labels each reusable daily dosage planner with all information required by M.G.L. c. 94C, § 21 for each medication.
