(1) A pharmacist shall maintain documentation or a statement of credit demonstrating successful completion of the required contact hours of continuing education for a period of at least two years from the date of completion. In order to demonstrate successful completion of required contact hours, a pharmacist must maintain documentation or statements of credit that contains the following information:

1. (a) the name of the authorized provider;
2. (b) the participant’s name;
3. (c) the title and number of the continuing education program;
4. (d) the date of completion of the program; and
5. (e) the number of contact hours of continuing education earned.
