Payment for the services described in 130 CMR 464.000 is made only to providers who are participating in MassHealth as of the date of service. The eligibility requirements are as described in 130 CMR 464.404.

1. (A) The PACT provider must be physically located within the Commonwealth of Massachusetts.
2. (B) The PACT provider must be certified to provide PACT services by the Department of Mental Health.
3. (C) The PACT provider must be enrolled and actively participating with the MassHealth agency as a billing provider as evidenced by the issuance of a provider identification and service location (PIDSL) number for the provision of PACT services at the PACT provider’s licensed, community- based location.
