Nonpayable services include, but are not limited to, the following:

1. (A) the chiropractic treatment of diseases and pathological disorders other than those related to a neuromusculoskeletal condition (including, but not limited to, rheumatoid arthritis, muscular dystrophy, multiple sclerosis, pneumonia, and emphysema);
2. (B) chiropractic manipulative treatment, office visits, and radiology services not personally provided by the chiropractor, in accordance with 130 CMR 450.301: Claims;
3. (C) chiropractor services provided in settings other than the chiropractor's office, or in the case of a MassHealth CHC provider billing for chiropractor services provided on site by a CHC-employed or contracted chiropractor, at a CHC. Nonpayable settings include, but are not limited to, inpatient or outpatient hospitals, nursing facilities, rest homes, and the member's home;
4. (D) laboratory services;
5. (E) orthotic devices, corrective devices, and orthopedic appliances;
6. (F) research, or experimental, unproven, or medically unnecessary procedures or treatment;
7. (G) maintenance therapy, that is, chiropractic manipulative treatment to maintain health or prevent regression when the member is no longer suffering or presenting symptoms;
8. (H) supportive services such as, but not limited to, nutritional counseling, educational services, and printed materials;
9. (I) physiotherapy, physical therapy, muscular stimulation, heat packs, or massage; and
10. (J) vitamins, minerals, food supplements, or other such supplies.
