allowable fees for substance use disorder treatment services. Payment is always subject to the conditions, exclusions, and limitations set forth in 130 CMR 418.000. Reimbursement for a service will be the lower of the following:

1. (A) the maximum allowable fee listed in the applicable fee schedule pursuant to 101 CMR 346.00: Rates for Certain Substance-Related and Addictive Disorders Programs, and 101 CMR 444: Rates for Certain Substance Use Disorder Services; or
2. (B) the program's usual and customary fee.
